The following fully insured HMOs were offered by Citi for 2008-2013 in each state. The inclusion of an HMO in a state list does not mean that the option is currently available throughout the state. Your home zip code determines if you are eligible to participate in one of the HMOs offered. You can determine whether the HMO is available where you live by calling the phone number on the HMO information sheet.
